I was born on August 18, 1963 at Good Samaritan in Phoenix,Az. My adoption was finalized around 1967 in Tucson, Pima County, AZ. When my parents applied for my original birth certificate, the surname "Flack" appeared to have been errased. It is believed that my birth mother had suffered a nervous break-down as she was in the Az State Mental Hospital at the time of my birth. The case worker assigned to monitor my adoption alluded to the possibility of my having several sibblings and that I was a mid-life baby. This may have placed some strain on my birth mother's mental state and may have caused my birth father to have abandoned the family. I am a 6 foot tall caucasion, fair complected with brown hair and have been slender all of my life.
