Cohen,Jill m. ( adopted name) Born 01/23/1980 at St. Bernardine Hospital at 4:23am. Dr. Edward Smith delivered me. Adoption Form Children's home society I believe. Was adopted at birth as far as I know. By Arleen and Joe Cohen. I had Brown eyes and black hair at birth. Looking for medical records due to a history of cancer on My husbands side of the family So that we have a better idea of what level of risk our daughter will be at. I know that there was a history of Diabetes in my birth family. Open to meeting up but would really like the medical records with no strings attached. Just would really like to have those for our little girl.
