My name after adoption is Edward Andrew Fendick From the adoption decree: Lake County Court, Lake County, Illinois, No. 24184; filed February 1, 1961; Judge Minard E. Hulse; court-appointed guardian was Andrew A. Semmelman My name was Baby boy Edler, born June 25, 1960, Waukegan Illinois “…that Judith Edler is the mother and sole surviving parent of the said child…” A receipt, dated June 29, 1960, for a payment from Richard Drew, Attorney, on behalf of Judith Edler (receipt is mis-spelled as “Ebler”), Room 364, to St. Therese Hospital, Waukegan Illinois. (probably paid from mom & dad to the attorney) Richard J. Drew Attorney at Law 4 S. Genesee Street Waukegan, Illinois On February 13, 1961, a birth certificate (“Filed for Record on February 7, 1961) was filed in my new name, Child’s Birth # 112-60-060208 Thanks for any help!
